In accordance with the Federal Law No. 152-FZ "On Personal Data" dated 27.07.2006, I hereby freely, voluntarily and in my own interest give my unconditional consent to the processing of my personal data by INFOTECH LLC (OGRN 1038600549283, INN 8602235879), registered under the laws of the Russian Federation at the address: 8, Bystrinskaya D., Surgut, 628406, Russia, Khanty-Mansiysk Autonomous Okrug - Yugra Autonomous Okrug (hereinafter referred to as the Operator).

Personal data - any information related to a specific or determined on the basis of such information natural person.This consent is given by me for the processing of the following personal data:
- Name;
- Telephone;
- Company name;
- E-mail;

This consent is given to the Operator to perform the following actions with my personal data using automated means and/or without using such means: collection, systematisation, accumulation, storage, elaboration (updating, modification), use, depersonalisation, as well as any other actions stipulated by the current legislation of the Russian Federation both by non-automated and automated means.

I hereby give my consent for the Operator to process my personal data for the following purposes:- to provide me with services/works;- to send me notifications regarding the provided services/works;
- to prepare and send replies to my requests;
- to send me information, including advertising information, regarding events/goods/services/works of the Operator.

This consent is valid until the moment it is withdrawn by sending a notice to this effect to the e-mail address oshmelev@rit.surgut.ru. In case I withdraw my consent for processing my personal data, the Operator has the right to continue processing my personal data without my consent on the grounds specified in paragraphs 2 - 11 of Part 1 of Article 6, Part 2 of Article 10 and Part 2 of Article 11 of the Federal Law Russian Federation "On Personal Data" No. 152-FZ dated 27.07.2006.
